It is important that our patients and families understand the journey of their loved ones when planning rehabilitation.

REFERRAL & ADMISSION:

 

1. What is the process to refer and admit a patient to Greentree Subacute and Rehabilitation?

a. The referring Doctor’s rooms should please complete and send the Greentree Health admission request form, together with the Doctor’s motivation letter, to reception@greentreehealth.co.za, and the relevant hospital Case Manager. The admission form can be obtained from reception@greentreehealth.co.za.

i. The following information should be provided on the admission request form please:

1. Patient’s details (patient hospital sticker with Medical Aid information, ID, telephone number, email address and physical address)
2. Hospital and ward the patient is currently residing
3. Patient’s diagnosis
4. Required length of stay
5. Special needs
6. Referring Doctor’s details

ii. The referring Doctor’s motivation letter should please accompany the admission request form. The motivation letter should address mobilisation, rehabilitation and physiotherapy requirements for the patient to gain independence again.

b. The Greentree Health team will then assess if the patient can be admitted.
c. If the patient can be welcomed and accommodated at Greentree Health, the Greentree Nursing Manager will contact the hospital ward and send the necessary documentation through to be completed by the ward.
d. The Greentree Case Manager will liaise with the patient’s medical aid and the hospital Case Manager to obtain authorisation for the admission.
e. Once medical aid authorisation is obtained from the medical aid, the hospital discharge and transfer process to Greentree Health can commence.
f. The hospital ward should arrange an ambulance service to transport the patient from the Hospital to Greentree Health. The Greentree Nursing Manager will communicate with the ward regarding requirements for the transfer. Ambulance services are usually covered by the medical aid if arranged by the hospital before/upon discharge.
g. The medication prescription should be provided to the patient by the treating Doctor. TTO medication should be obtained and sent with the patient to Greentree Health. The Greentree Nursing Manager will communicate with the hospital ward regarding the TTO medication.
h. The patient is then discharged from the hospital and transported to Greentree Subacute and Rehabilitation via ambulance.
i. The patient is welcomed upon arrival at Greentree Health, admitted and assessed, so care and treatment can commence.
j. Feedback to a patient’s referring Doctor is done on a frequent and continuous basis during their stay. The Nursing Manager and her team liaises with the Doctor via telephone and WhatsApp, and emailed reports are provided to the Doctor’s rooms on a regular basis.

2. What forms should be completed when referring a patient?


a. The Greentree Health admission request form should be completed and sent to reception@greentreehealth.co.za, together with the referring Doctor’s motivation letter. The admission request from can be obtained from reception@greentreehealth.co.za.


Essential items that our patients should bring along, in order to expedite the admissions process:

  • ID card of patient as well as main member of medical aid
  • Medical aid membership card
  • Medical reports, X-rays and doctor’s letters
  • Medication prescribed and provided by hospital and current chronic medication
  • Physical aids (hearing aids, walkers, crutches, etc.)
  • Sleepwear, dressing gown and slippers (comfortable clothing)
  • Toiletries and personal effects
  • Books and magazines

3. How should a patient be transported from an acute hospital/day clinic to Greentree Health?

a. The hospital ward should arrange an ambulance service to transport the patient from the hospital to Greentree Health. The Greentree Nursing Manager will communicate with the ward regarding requirements for the transfer. Ambulance services are usually covered by the medical aid if arranged by the hospital before/upon discharge.

4. Does Greentree Health do, or require a COVID test before admission?

a. Yes. A COVID-19 test should be done and not be older than five (5) days. Unfortunately, no positive COVID-19 patients can be admitted. The Greentree Nursing Manager will contact the hospital ward and send the necessary documentation through to be completed by the ward.

5. What is the fee per night?

a. Private patients’ fee: R2,530.00 per night.
b. Medical aid tariffs apply for medical aid patients.

6. Do you have a doctor at the facility/doctor on call?

a. Greentree Health has a General Practitioner (GP) on call to assist patients.
b. Referring Doctors are also welcomed to come and see their patients at Greentree Health.
c. The Greentree nursing team can assist to arrange a follow-up appointment with the referring Doctor.
d. If a patient complicates, our Paramedics-on-Call services are dispatched and the patient is taken to the hospital where they were discharged from. If this is not possible, the patient is taken to the nearest hospital’s emergency unit for emergency assistance. The referring Doctor will be informed by our nursing staff on duty.

7. Regular physiotherapy is a key requirement as part of a subacute and rehabilitation facility. Do you have physiotherapists available?

a. Yes. Greentree has a team of physiotherapists available to assist patients on their road to rehabilitation and independence.
b. Specialists can also have their specific physiotherapists treat patients at our facility.
c. Greentree Health assesses the acuity of a patient’s condition and compiles a motivation letter, which is sent to the medical aid should additional physiotherapy be required. Close communication with the referring Doctor is managed continually.
d. A psychologist, occupational therapist, speech therapist and dietician can be arranged if required. These services will be for the patient’s own account.

8. Are communication channels in place and updates provided to referring Doctors?

a. Feedback to a patient’s referring Doctor is done on a frequent and continuous basis during their stay. The Nursing Manager and her team liaises with the Doctor via telephone and WhatsApp, and emailed reports are provided to the Doctor’s rooms on a regular basis.
b. Referring Doctors are also welcomed to come and see their patients at Greentree Health.
c. The Greentree nursing team can assist to arrange a follow-up appointment for the patient with the referring Doctor.
d. Close communication with the referring Doctor is managed continually.

9. Can IV antibiotics be given to patients? What must be brought with?

a. Yes. Greentree Health assist patients who need to receive IV antibiotics.
b. The required medication with drips should please be brought with the patient.
c. We prefer that patients are transferred from the hospital to Greentree Health with lines in situ.
d. Greentree Health nursing staff can care for patients with CVP lines (preferred) or ports, if needed.
e. Our nursing staff are also skilled and equipped to do ECGs if needed.
f. Greentree Health can arrange blood tests with Pathcare, where required.
